NOTICE OF INHERENT RISKS: I fully understand that there are inherent risks associated with Equestrian Activities and horse shows. Horses: have the propensity to behave in ways that may result in injury, harm, or even death to persons on or around the horse; have unpredictable reactions to such things as sound, sudden movement and unfamiliar objects, persons or other animals; and are susceptible to certain hazards, such as surface or subsurface conditions, or collision with other horses or objects. Horses also have propensities which include kicking, biting, stamping, stomping, stumbling, rearing and similar actions. Equestrian equipment can fail resulting in falling or loss of control. Equestrian Activities also have the potential of a participant acting in a negligent or careless manner that may contribute to injury or death to the participant or others, including spectators, such as failing to remain in control over the horse, or not acting within the • participant's ability. I fully understand that equine/horse activities are INHERENTLY DANGEROUS. Accordingly, I am knowingly assuming all risks related thereto, and waiving any and all liability of Maui Meadow Farm, its owners, partners, employees, representatives and agents (collectively called "Maui Meadow Farm"). WARNING: YOU ASSUME THE RISK OF EQUINE ACTIVITIES PURSUANT TO PENNSYLVANIA LAW. Under Pennsylvania law an equine professional and equine activity sponsor is not liable for an injury to, or death of, participants in equine activities resulting from the inherent risks of the Equine Activities (see Equine Activity Immunity Act, Act 93 of2005; 4 P.S. §§601-606). I, the undersigned, having read and understood the content and terms of this document, AGREE and CONSENT to the all of the provisions contained herein. It is my intention and desire to attend and/or participate in equestrian related activities being conducted, or otherwise sponsored by Maui Meadow Farm, which equestrian/horse activities include but are not limited to riding, showing, jumping, handling, mounted games and/or combat, marshaling, working on the ground crew, training, volunteering, viewing, observing and/or spectating (all of these activities are collectively called "Equestrian Activities"). I acknowledge that the Equestrian Activities are inherently and potentially dangerous, and I voluntarily accept all of the inherent risks involved for me, and the minor children for who I am responsible. I understand that in consideration for my being permitted to take part in the Equestrian Activities, that I am fully and completely waiving and releasing any and all liability which Maui Meadow Farm may hav􀀚, which is in any way associated with the Equestrian Activities, even if Maui Meadow Farm is found to be negligent, grossly negligent, or otherwise intentionally responsible. Further, I understand and agree that this waiver and release is intended to be binding upon myself, my heirs, executors, and assigns. I also agree to release, hold harmless and indemnify Maui Meadow Farm from and against all claims, actions, suits, demands, costs and expenses, of any kind whatsoever, with respect to death, injury, loss or damage to my personal property, however caused, arising out of, or in connection with, my taking part in the Equestrian Activities. I agree to pay all costs and expenses, including attorneys fees, incurred by Maui Meadow Farm, or its respective insurance companies, incurred in the defense of any such claim, action or demand. I further agree that if I am a parent or guardian of a minor who is participating in any of the Equestrian Activities, I consent to that child's participation, and agree, and assume, all of the provisions of this document on behalf of the child.

I UNDERSTAND THAT THIS IS A LEGAL DOCUMENT. I HAVE READ AND UNDERSTOOD THAT THIS CONSENT AND WAIVER, AND I UNDERSTAND ALL OF ITS TERMS. I EXECUTE IT VO LUNT ARIL Y AND WITH FULL KNOWLEDGE OF ITS MEANING AND SIGNIFICANCE. I HEREBY ASSUME ALL THE RISKS ASSOCIATED WITH THE EQUINE ACTIVllCIES. l AM SIGNING THIS DOCUMENT INTENDING TO BE LEGALLY BOUND HEREBY.
